Complete rest is not usually the goal with Achilles tendinopathy. Activity is commonly modified while the tendon is gradually strengthened and its capacity to tolerate load is rebuilt.
Can you keep walking?
Often yes, if walking is tolerable and does not cause a significant worsening of symptoms. You may need to reduce distance, speed, hills or prolonged standing while the tendon is irritable.
What about running?
Running may need to be temporarily reduced or paused if it causes a large symptom flare. Return should be gradual and matched to the tendon’s improving strength and tolerance.
What exercise is useful?
NHS physiotherapy guidance places progressive calf and Achilles strengthening at the centre of treatment. Rehabilitation may progress from easier heel-raise exercises to more demanding loading as symptoms improve.
Can low-impact exercise help maintain fitness?
Activities such as cycling or swimming may help maintain fitness while reducing some of the impact associated with running and jumping, provided they are comfortable for you.
Is some discomfort during rehabilitation normal?
Mild discomfort can occur during tendon-loading exercises. The important point is how symptoms respond afterwards and the following day. Significant or progressively worsening pain suggests the programme needs modifying.

A heel lift may temporarily reduce tendon strain for some people and can be used alongside rehabilitation, but it should not replace progressive strengthening.

Achilles tendinopathy can take months to improve. NHS guidance notes that many people need a prolonged rehabilitation period before returning fully to pain-free activity.
Stop and seek prompt assessment if you develop sudden sharp pain, hear or feel a snap/pop, lose push-off strength, cannot walk normally or cannot stand on tiptoe. These symptoms may indicate rupture rather than tendinopathy.
