When might a knee immobiliser be used?

A knee immobiliser may be used when restricting movement is an important part of protecting the knee. This can include some injuries, fractures, post-operative situations or periods following removal of other forms of immobilisation.

The exact reason for immobilisation will determine how long the support is required and whether weight-bearing or walking is appropriate.

When might a ROM knee brace be used?

A ROM brace may be considered when the knee still requires substantial support, but completely preventing movement is no longer the aim.

Adjustable hinges allow rehabilitation to take place within defined movement limits. Those limits may sometimes be altered as healing and rehabilitation progress.

If you have specifically been told to use an immobiliser, check before changing to a different type of brace.

Is a hinged knee brace the same as a ROM brace?

Not always. A standard hinged knee support typically uses hinges to provide medial and lateral stability while allowing relatively natural bending and straightening.

A ROM brace adds the ability to restrict or control that movement through adjustable flexion or extension settings.

This makes ROM braces particularly useful when the permitted movement of the knee forms part of a rehabilitation plan.

Which brace is used after knee surgery?

There is no single brace used after every knee operation. Some procedures may require no brace, while others may involve an immobiliser, ROM brace or another specialist support.

Where a brace has been prescribed, its purpose and settings depend on the operation performed and the stage of rehabilitation.

Follow the post-operative instructions provided by your surgical or rehabilitation team rather than choosing a brace based only on the type of surgery.

Can you walk in a knee immobiliser?

Whether walking is appropriate depends on the injury and any weight-bearing restrictions you have been given.

Wearing an immobiliser does not automatically mean it is safe to place full weight through the affected leg. If you have been given crutches or specific weight-bearing instructions, continue to follow them.

Can you walk in a ROM knee brace?

ROM braces are commonly designed to allow mobility while the knee remains supported, but the permitted level of walking and weight-bearing is still determined by the underlying injury or operation.

A brace can control movement at the knee, but it cannot determine how much load injured or healing tissues are ready to tolerate.

Which is more restrictive?

A knee immobiliser is generally the more restrictive option because its purpose is to minimise knee movement.

A ROM brace is designed to provide more flexibility by allowing movement within selected limits.

How should each brace fit?

Knee immobiliser

The immobiliser should sit securely around the leg with its supportive stays correctly positioned. Straps should hold it firmly without causing numbness, tingling or excessive pressure.

ROM knee brace

A ROM brace also needs its hinges correctly aligned alongside the knee. The brace should remain in position when you move rather than sliding down or rotating around the leg.

If your ROM brace includes telescopic side bars, these may also require adjustment to suit the length of the leg.

What should you check during wear?

Skin

Check for persistent redness, rubbing, pressure areas or skin irritation beneath straps and supportive sections.

Circulation

Numbness, tingling, marked swelling or changes in skin colour or temperature should not be ignored.

Position

Make sure the brace has not slipped, rotated or moved away from its intended position.

Settings

With a ROM brace, check that movement stops or locking settings remain as prescribed.

When might you change from an immobiliser to a ROM brace?

During some rehabilitation programmes, the need for complete or near-complete immobilisation may reduce as healing progresses.

At that point, controlled movement may become appropriate and a ROM brace may sometimes be introduced.

The timing of this transition depends on the injury or operation and should be guided by the healthcare professional responsible for your rehabilitation.

When should you seek professional advice?

Seek advice if you are unsure which type of brace has been recommended, do not understand your ROM settings or cannot achieve a secure and comfortable fit.

You should also seek help if you experience increasing pain, substantial swelling, numbness, circulation changes, skin damage or a significant deterioration in your symptoms.

Choosing between a knee immobiliser and ROM brace

The key question is whether the knee needs to be immobilised or whether controlled movement is appropriate.
