What does ROM mean on a knee brace?

This allows the knee to remain supported while movement is gradually introduced or controlled during rehabilitation.

Important: adjustable ROM settings are not intended to be changed simply according to what feels comfortable. If a brace has been prescribed following an operation or significant injury, use the settings provided by your clinical team.

When might a ROM knee brace be used?

After knee surgery

A ROM brace may be prescribed after certain operations where knee movement needs to be controlled during the early stages of rehabilitation.

How is a ROM brace different from a normal hinged knee brace?

Both types use hinges to provide structure around the knee, but their purpose can be different.

ROM knee brace vs knee immobiliser

A knee immobiliser and a ROM brace are not the same thing.

An immobiliser is intended to restrict movement of the knee. A ROM brace, by comparison, can allow movement within defined limits.

This can make a ROM brace useful during stages of recovery where some knee movement is appropriate, but completely unrestricted movement is not.

Which type is required depends on the injury, operation and stage of rehabilitation, so one should not simply be substituted for the other without appropriate advice.

How do the adjustable hinges work?

ROM hinges allow limits to be placed on flexion and extension. Flexion refers to bending the knee, while extension refers to straightening it.

The exact adjustment system varies between braces. Some use pins, stops, dials or locking mechanisms that allow the permitted range to be changed.

The purpose is not necessarily to stop movement completely, but to prevent the knee moving beyond the range considered appropriate at that stage of recovery.

Who decides the ROM settings?

Where a ROM brace forms part of post-operative or injury rehabilitation, the permitted movement should normally be decided by the surgeon, physiotherapist or other healthcare professional responsible for treatment.

Settings may change as recovery progresses. For example, a knee may initially have a relatively restricted range before gradually being permitted more movement.

Do not increase the permitted movement simply because the knee feels better unless you have been advised that it is appropriate to do so.

Can a ROM knee brace be locked straight?

Some ROM braces include a locking function that can hold the knee in a particular position, including full extension, depending on the design of the brace.

This can be useful when movement needs to be restricted at a particular stage of treatment. However, locking positions should be used according to the fitting instructions and clinical plan rather than selected without guidance.

Can you walk in a ROM knee brace?

ROM braces are often designed to be worn while mobile, but whether you should walk and how much weight you should place through the affected leg depends on the injury or operation.

A brace controlling knee movement does not automatically mean that full weight-bearing is safe.

If you have been given crutches or specific weight-bearing instructions, continue to follow those instructions while using the brace.

How should a ROM knee brace fit?

The brace should be positioned so that its hinges align correctly alongside the knee. Thigh and calf straps should hold the brace securely without causing excessive pressure.

The brace should not repeatedly slide down the leg or rotate during normal movement. Poor positioning can affect both comfort and how the hinges move with the knee.

Telescopic ROM braces may also allow the length of the side bars to be adjusted to suit the leg.

What if the ROM brace keeps slipping?

Check that the brace has been applied in the correct position and that its straps have been tightened evenly.

Overtightening the straps is not a good solution to persistent slipping. If the brace continually migrates despite being correctly fitted, seek help with the sizing or fitting.

Should you sleep in a ROM knee brace?

Whether a ROM brace should be worn overnight depends on the reason it has been prescribed.

Some post-operative treatment plans may require a brace to remain in place for extended periods, while others may have different instructions.

Do not remove a prescribed brace for sleeping, or decide to wear one overnight, based on general advice alone. Follow the instructions provided by your clinical team.

How long is a ROM brace usually needed?

There is no standard period that applies to every person. The duration depends on the injury or operation, healing progress, knee stability and rehabilitation plan.

As recovery progresses, the permitted range of motion, wearing schedule or type of brace may change.

This is one reason ROM braces are particularly useful in rehabilitation: one brace can provide different levels of movement control at different stages when clinically appropriate.

What should you check while wearing a ROM brace?

Hinge position

Check that the hinges remain correctly positioned alongside the knee rather than gradually moving up or down the leg.

Strap pressure

Straps should be secure without causing numbness, tingling, excessive pressure or circulation problems.

Skin condition

Regularly check for rubbing, pressure areas or persistent redness, particularly during longer periods of wear.

ROM settings

Check that adjustable stops, dials or locks remain at the settings you have been instructed to use.

When should you seek help?

Contact the healthcare professional managing your treatment if you are unsure how your brace should be set, cannot achieve a secure fit or believe the prescribed settings have changed accidentally.

You should also seek advice if you develop increasing pain, significant swelling, numbness, changes in circulation, skin damage or symptoms that are worsening rather than improving.

Choosing a ROM knee brace

ROM braces vary in length, hinge design, adjustment method and fitting system. The most important consideration is whether the brace provides the type of movement control required for your stage of rehabilitation.
